Healthcare for the Homeless - Houston's (HHH) mission is to promote health, hope and dignity for those affected by homelessness through accessible and comprehensive quality care. HHH exclusively provides care for individuals who are affected by homelessness or are unstably housed.
Our clinical model and services meet the distinct, and often complex, health and social needs of those we serve. Our purpose is to be a primary care "health home" for those experiencing homelessness. Our integrated healthcare is centered on co-located medical care, dental care, mental healthcare, substance use counseling and intensive case management. Additionally, HHH operates innovative community outreach programs that link those experiencing homelessness with resources critical to their overall health, including Permanent Supportive Housing.
